If you are unable to attend or wish to reschedule your appointment, please provide at least 24 hours' notice; otherwise, a cancellation fee may apply.
All correspondence with this office is kept confidential and safe. Please do not disclose your appointment or personal information with anybody outside of this office.
For In-person appointments Please arrive 15 minutes before your planned appointment time to complete any necessary papers and be settled before the session begins.
If you are going to be late for your session, please phone our office as soon as you can. We will do our best to accommodate late arrivals, but please plan accordingly because it will affect the length of your session.
If you have insurance, please bring your policy details, including the name of the insurer, policy number, and subscriber ID numbers.
Please include an emergency phone number in case of necessity during the session.
For Telemedicine, Clients are responsible for ensuring that they have a stable internet connection, a compatible device (computer, tablet, or smartphone), and a quiet, private space for video consultations.
