Booking Terms:
On your first visit to the clinic, please, bring a photo ID
Can be any document with your picture on.
If you are making a booking for ANOTHER PERSON, please, leave the patient details on 'NOTES' field.
If you are booking for an appointment for CHILD:
**If you are a parent or guardian, please ensure to include the child's name and date of birth in the 'NOTES' field during the next step of the booking process.**
Please, bring the child 's red book and photo ID .
If you arrive more than 10 minutes late for your appointment, the doctor might not be able to see you.
Or you may have to wait until a gap on Doctor's schedule becomes available.
INFORMAÇÃO IMPORTANTE:
PT: Se o agendamento for para outra pessoa, (amigos ou parentes) por favor, deixe os detalhes do paciente, (nome e data de nascimento, email e telefone) no campo ' ADD NOTES'.

CANCELATIONS POLICY : 24hrs notice is required to cancel appointments, otherwise REFUNDS will not be possible.
REFUND POLICY: Refunds are made only for Cancelations within 24hr- NO REFUNDS will be done for requests of CONSULTATIONS ALREADY TAKEN.
